Survey questions not attached to email recipient

I just discovered the survey I sent at 6am - which I also sent to myself  - does not seem to have the actual survey attached.  The message in my own inbox, is my prepared header - stating there is a survey and thanking people for taking some time to complete it - -but nothing to complete.  Any ideas what I did wrong or why this failed me?  I would like to correct this and resend it asap.  Not able to reach the constant contact support people for a couple hours.

We experienced the same issue. But we changed the line that says something like  'Take the survey now' to 'Take....' So if we clicked on  'Take' it took us to survey. We thought we could change that line.
