The CCSFG only references the list name internally in its own configuration. It uses this reference to gather the appropriate ID that is passed into your account so that the contact is added correctly. It sounds like one of the configuration files is not allowing the new changes to be rewritten. I would suggest downloading a fresh copy of the CCSFG and then uploading it to a new directory. That way it will be able to take your new list values.

But judging from Richard's reply it appears to be working for him but not for me.
When a contact has been added successfully to your list, the form tries to take them to a page -- thankyou.html -- to thank them for joining. The problem is the thankyou.html file is not on the server. So, if you place that file in the CCSFG folder, the second form will work.
To illustrate, visit http://www.boomkinproductions.com/SP/CCSFG/index.html and type in an address (something random) that isn't on your list and then submit the form. Then, log into your Constant Contact account and click on Contacts. Then, type in an address in the search box and click on Search and you will see that the contact is now on the Golf_Sweepstakes_HT list.
I hadn't tested the first form (http://boomkinproductions.com/SP/CCSForms/) earlier, but I just did and received the following message: "THANK YOU! You will be notified if you are a winner." I logged in to your account and found that I was now on the Golf_Sweepstakes_HT list..
So, to recap, the second form appears to work, but just displays an error message because it can't findthankyou.html . The first form appears to work and not display an error message. What happens when you try to join using the http://boomkinproductions.com/SP/CCSForms/ form?
Also, I should mention that the contacts are being added to the mfarmer1 account. I noticed that your company has two accounts, so that's why I mention that.

But I just had not set it up yet for other form. This is easy to remedy, however neither form is actually working for me, in that, it is not posting the record to CC. I have tested both thoroughly, from different browsers and on different devices but the information is not posting to the CC list. Do you know why this might be happening, that it would work for you but not for me? Thank you for any insight you may be able to provide.
(I am working in the mfarmer1 account and attempting to post to the "Golf_Sweepstakes_HT" list. As you are likely able to see, the only test records in there are yours and I have been testing it without success.)

Is it possible to have these two forms deliver their records to different lists? In other words, I would like:
...CCSFG/index.html to deliver record to "Golf_Sweepstakes_HT"
and
...CCSForms/index.html to deliver record to "SweepsHPOC"
If so, I will make the necessary changes to the code. I just wanted to be sure, before I tried it. Thank you.

Not a problem. In the CCSFG and CCSForms folders there is a config.php file you can edit in a text editor. Line 6 should look like this:
var $contact_lists = array("Golf_Sweepstakes_HT");
If you wanted them to join the SweepsHPOC list instead, the code would look like this:
var $contact_lists = array("SweepsHPOC");

The form at http://boomkinproductions.com/SP/CCSFG/ does appear to be working. The only issue is that the success page -- http://www.boomkinproductions.com/SP/CCSFG/thankyou.html -- does not exist. So, if you create that page, the form should work more smoothly.
