Help Getting Sign-up form to work using the Form Generator

I'm trying to make a sign-up form work with form generator and need your help.

I did everything right but when I test the form, it goes to the error page, and does not add people to my list.

Hey Rob,

I took a look at your installation and do not see an issue.  I also commented out that part of your post as i dont want anyone to point to it for security reasons.  If you can please supply me with your code that is generated so i can troubleshoot it that would be great.
