Two Identical Forms on Same Page

I have this form:

This form is used at two differenct places on the home page, one in the main content area and the other in the footer. 


Keep in mind that both forms are indentical. When both forms are being used the first form works fine but the second does not.  If I remove the first form then the second works fine.

Any suggestions is greatly appreicated.





I tried your site and they are both working for me.  I think you may be experiencing an issue with various browsers because of the same form name and input names.
