trying to use constant contact ccsfg on our server and getting perpetual error page

Hi,

I thought I'd start a new thread since the old one was... well.... old.  I'm trying to implement CCSFG on a windows server running php 5.0 with inherited permissions in the IUSR account but getting error pages when testing (http://www.houseofguitars.com/curts-list.html)

Any ideas?

I tested out your form and it looks like it is successfully working right now (I was able to sign up to your list).  Are you still having problems or did you figure it out?  If you figured out the issue, it would be greatly appreciated if you shared so anyone else running into this is able to take advantage of your solution.
