Activate and install an inline or pop-up contact sign-up form on a website

Does anyone know how to correctly configure this for a FaithConnector website?  I reached out to them for help, and they were unable to assist.

Hi, these were the initial steps that I followed to try and install the in-line form on my church website's page.  Unfortunately, I was still unable to get the inline form to appear.  The issue, I believe, is that the software we use (FaithConnector) is templated web pages, and I can't directly access major sections of the pages' HTML code.  So I'm really struggling with how these forms work on my particular type of website.
