Adding a contact with SMS consent / opt-in

I just signed up with constant contact.  Speaking with the sales person on the phone I was told that I could manually add a contact and manually opt-in the contact for SMS.  All of my contacts are patients at my clinic that opt-in to SMS on a paper form.  I have added a few test contacts though am unable to send SMS to them.  Constant Contact says they need to fill out an opt-in form to receive SMS.  Am I missing something?  Is there any way to manually opt in a contact for SMS?

The one way to get your existing contacts signed up for SMS would be to send them a link to a sign-up form here.
