How to share a Survey Page so that results are NOT anonymous?

I need to do a survey and harvest results of who answered.  NOT anonymous.  How do I ensure that?  It states that depending on how it's shared some results will be anonymous.

To make sure the results of your survey are not anonymous, make sure to choose to share the survey with an email. Click here to see what to select. Thanks!
