Hi, I copied and embedded the code from Constant Contact into the home page of a web page (see towards the bottom of the home page by the footer) - http://fightbac.staging.wpengine.com/ For some reason, the state field shows up three times. I can't figure out why since in the code I cut and paste in, it only show up three times. Wondering if you can help me out with this. Thank you!

I am very sorry the code is causing you to have three state fields! I took a look into the account, and unfortunately, it looks like the code you copied into the website has been modified from the original code. I recommend copying and pasting the code, once you get the sign up form personalized in Constant Contact, back into the website. Then, check and see if the State field comes up correctly. If it is still an issue, let us know and we can help troubleshoot from there.
Hi I put the code in exactly as it was from Constant Contact without any styling, so you can see the fonts, etc. since everything is white, but you can see that the triple state field problem is still there, plus now country also shows up three times.

It's very strange because I do not see state three times in the embedded code in the php file. But if you do inspect element in Chrome, you can see the code listed three times so I'm totally baffled as to why that is happening!

We definitely want to get to the bottom of this. I did look at the page source for your website and compared the code embedded there with the embed code as it appears in your Constant Contact account and they were different. They do not share the same titles,

I was able to check with our upper level support team and they also looked at the code as it is embedded in your site. They said that the code as it is currently is fine and there's nothing in the code itself that is causing the state field to repeat. They even pulled the code from the website and tested it in another area and the state field did not repeat. They think some other code that makes up the website is interferring with our sign-up form code and causing this issue but unfortunately web HTML is a bit out of our purview. I encourage you to check with a web developer to see if anything sticks out!
