Survey page responses all anonymous

I have sent multiple surveys and email likes to my created contact email list. Each is subscribed however, when they target audience response I am constantly receiving "anonymous" responses. Customer service is not helpful with an explanation of why this is happening or how to turn this off.

Has the survey page been shared in any other way, besides as part of an email campaign? Are you aware of any of your contacts forwarding the email to other people? Have you tried quicksending the email to your own contact info in your account, to see if your response counts and displays your contact info in the reporting as expected?
