I see that there is a time lag between email opens and adding them to the tally on the cover of the report. When I click Opens and go to the list of all opens the list total is many more than what the summary has shown. Does the cover page of the report only refresh every 5 minutes perhaps, instead of in real time?

This is correct. Viewing the email's reporting pages will always have the most up-to-date reporting for opens and clicks, compared to the My Campaigns overviews.
