Dynamic content in emails

I would like to be able to have a conditional term. If a field is 0 or blank, do not print the following sentence. "You have ____ number of points available that expire on ____________." If my custom field of points and date are blank, I do not want the sentence to print. Or, if a custom field = Y, then print. This would eliminate having 2 lists and 2 emails.
