Enable custom surveys by adding contact info fields

I was hoping my first use of the survey tool would be an annual contact info update. I wanted to include the employer, phone, etc. in the questions and ask them to verify or update.  I can work around this by including the contact info in the email and having them ping back and forth, but it's not ideal.
