Thank you for reaching out to us through the Community. I am sorry to hear that you are experiencing this issue. Have you tried using another browser to see if it is not a browser-related issue? Also, could you provide a screenshot of the blank page that you are seeing? Are there any error messages that come alongside the blank page as well?
Thanks for quick response! On a different browser the forms do show. I am using a Mac with the latest operating system... this browser is the one that does not work correctly. If you have ideas regarding why, please let me know!
