What type of form with a download button are you looking for? While you could set up a Sign Up Form, it wouldn't automatically download. But you could attach that to an Autoresponder which would trigger an email with the file you want to be downloaded. You may also respond back to this message if you are looking for another option.

I would like it to look like this attached image. So the field for name, then email then the download button.
Taking a look at your screenshot, the sign up form that best fits would be our Inline forms. I do apologize for the inconvenience, as our forms do not have the option to put a download button on them yet. I would love to track your feedback on this! Our inline forms will be able to transfer your contacts information from your website over to Constant Contact automatically so you can start emailing them right away though. Thanks for the reply. I kind of figured as much but thought I would ask my question. I have another question. How do I keep from getting multiple email addresses? For instance let's say some one signs up twice on two different blog posts ... do I get their email in my systems twice?
That is a great question! If a potential customer signs up on two different blog post sign up forms, their email address will not be entered in your account twice. Our system will filter out the duplicates for you so you will not have to worry about them being entered twice.
