Multi Choice Questions

I would like to request the ability to place a comment or text box next to each answer of a multi choice multi answer question. For example...in my Real Estate Rental Survey... if someone were to choose Mem. Day to Labor Day it would be good to be able for the responder to put the amount right next to it... thanks!
