I downloaded my custom report for an event and asked for Guest Information, and for Attendance Information. If I had marked the Registrant as Attended, their guest(s) automatically show up as Attended as well, on the report. If I change the Registrant to Not Attended, the Guest Status also changes. It seems that if you cannot fix it so that Guest Attendance is recorded, then that field should be blank on the Report. Because it can be the case that the Registrant Attends, but not the Guest(s). Therefore Guest attendance field on the exported report should be blank.
