Email a T-Shirt Order Form

Hi Folks:

I know there must be a way to do this. I need to send our investors an order form they can use to get a free t-shirt with our company logo. We have about 2,000 (crowdfunded) investors so I can't just put an order form on the web (besides, folks will complain I am making them go to a website to do this).

Does CC have the capability of creating such a form and then dumping it to a list with custom fields (name, address, t-shirt size)
