I think the key question is not, "how likely are you to recommend us" but, "WILL you recommend us? Yes or No." If the answer is Yes, "who do you know who could use our services and products?" I believe a lot of people say they will recommend you, but never do. Other people say it's unlikely, but they end up sending business your way. Go figure. ;-) Some recent research has shown that NPS is not the magic number it was originally thought to be. I had been using Zoomerang for surveys. It does allow for 10 responses. The information I received from 10 options is no more useful than information from 5 responsse questions. In fact, some people get so frustrated try to break thinbgs down that finely that they end up blowing off the survey and never complete it. FWIW, the main reason I used to subscribe to Zoomerang (at $599 per year!) was to have "skip logic." With Constant Contact's survey service, I can have the same functionality, in most cases, for an additional $7 per month.
