Regarding the first question, what occurred was that your coding had all images set to be 100% sizing. For future custom code emails, do not do this, as it will cause our 1px tracking image to blow up. There's nothing in place to really alert to this occurring other than doing the recommended test send or preview prior to sendout. As the technical agent explained, this isn't normally an issue because it is generally not good practice to have coding that makes all images 100% sizing.
Per your second query, the tracking image cannot be turned off at all, because it is vital to our tracking and reporting systems doing their job.
Regarding your third question, I'd recommend reading this article on using our custom code editor. If you are using that editor to send out that type of email, then you'll obviously need to verify it looks good in our system before sending out.
