Non-profit membership

Our CC website needs a MEMBERSHIP PAGE. (We're a non-profit art organization.)

Customers would be able to CHOOSE/CLICK: 

Individual membership

Senior citizen

Student

Family

Corporate. 

Each membership is a different AMOUNT. So, once a membership level was chosen, the BUTTON for that membership level would go to a certain amount. Then, in addition, we absolutely MUST keep track of each of those new members, so they would need to fill out field forms for: 

name

address

phone

email

membership level 

interest in volunteering

Can this all be done with CC and if so how? Can we get a tutorial??

I would like to have the same thing, specifics: 

Membership BUTTON > Customer clicks TYPE OF MEMBERSHIP... Individual 

Senior

Student

etc.

(all different amounts).  > Then customer fills in FIELDS with: 

Name

address

phone

email

etc.

CAN THIS BE DONE ON CC WEBSITE?
