Adding box for phone number to opt in form
How can we add an optional field to this form to ask for a phone # Pilots To The Rescue : Sign Up to Stay in Touch (constantcontact.com)

It looks like that link goes to a legacy sign-up form, which can be edited in the Basic Landing Pages section of the Sign up forms tab.
I'd recommend switching to and using the updated landing page sign up forms available in your Campaigns.
