There is a header on my that is clearly custom and I cannot figure out how to get it off?

Hi @MelissaM273 ,
Unfortunately the unsubscribe form is not customizable. We do this to keep compliant with anti-spam laws. You can, however, edit the Update Profile form. I always suggest customizing this form, if you are wanting to give your contacts the option to opt in and out of certain lists.
