Based on your wording, I'm not sure if by "registrants" you're referring to event registrants, or you're referring to contacts.
When it comes to event registration, you can have multiple tickets (registrations) under a single primary registrant. Only the primary registrant's email is necessary.
When it comes to contacts, it's not really viable or a best practice with our system, but you can technically have multiple names listed under a single contact. Anything beyond the default First Name and Last Name fields would need to be custom fields you make for those other family members.
