Choose menu for survey

If I am setting up a luncheon and want to send a survey with food choices, how can I add more than one survey that allows more than one selection for a variety of menu options? I.e., choose a sandwich meat with no cheese and choose condiments.

At this time, there wouldn't really be a way to do this, without just including every option in the initial survey, and offering a "not applicable" choice to some of the questions. For example:

What kind of cheese do you want for your sandwich (select all that apply)?*

• Cheddar
• American
• Swiss
• Not Applicable
