I recently deactivated my Constant Contact subscription, yet the signup form remains on my website. Could you guide me on how to remove it?

We wouldn't have any insight or ability to remove that coding from your site. I'd recommend reaching out to your site builder's support or webmaster for assistance removing the sign up form's code from your site.
