Testimonial Collection form
Hi, I would love to see you create a landing page with input fields for a testimonial collection form very similar to this one my friend uses! It would send the input to either a doc or spreadsheet, or each time by email perhaps, so we the practitioners/business folks can post the words from our clients/customers wherever we need them to be.
Ideally a place for folks to upload a headshot, if desired, would be nice.

Hi @LuluM2 You can have custom fields to collect information on a landing page but the fields would need to be manually exported. A survey page would work best in this case for receiving feedback or testimonials since there are options for open-ended questions where clients have a lot of space to talk about their experience.
