How do I add a custom field to a Facebook lead form?

I was on with the CHAT people and cannot get an answer, I need to add a FIELD to the Facebook Lead Form so I can identify 

what ADD the person clicked from, since all contacts go to ONE LIST, Facebook LEADs

So I need TO ADD A CUSTOM FIELD to This FORm How do I do IT??
