Greetings.

I received an email from our CC contact form but the new contact is not successfully added to our specified list.  

Looking at the logs it appears this has happened in the past. 

Here is the recent log error.

Please note, there are no custom fields - Just first name, last name and email.

It looks like you are using the integration “Constant Contact Forms for WordPress”, which was developed by and is directly supported by Constant Contact, so I’m going to escalate this case to one of our Tier 2 support specialists so that they can follow up directly for further troubleshooting.
