Empty iframe being generated in embedded form causing error on Safari (desktop and mobile)

Hi:

I am getting a security error for an iframe that has no source associated with it when embedding the form. The security error only shows up in Safari

I can't find any settings that remove that iframe.

Any ideas as to what I can do?
