You'd just need to remove the universal code from your site. If you need assistance on accessing your website's code, make sure to reach out to your webmaster or the website builder's support.
Otherwise you can also deactivate the pop-up form in your account, though the code would obviously still be in your site until removed.

I use blogger and there is no support.
We don't have any insight on other systems' website builders, nor the ability to access your website to remove that coding. So you'll need to go into the coding, find the universal code, and remove it.
