contact csv upload not working for me?

I have an Excel csv file with headers Name, Phone and Email that I have not been able to upload.  It keeps prompting me to import an Excel csv etc file....... which I have numerous times?  What am I doing incorrectly?

Did you submit a callback when you called in, or have you tried replying directly to the automated @ mention email you received from my response?
