When responding to the survey, it's not clear that it was submitted because the page just sits there.

How to 1) have it say thank you, or 2) have it go back to the original email after submitting the answer?

Once a survey is successfully submitted, it will redirect to the default (non-customizable) thank you page. If it doesn't redirect, then chances are something connectivity-wise is preventing the info from being submitted.

Does the survey response work in the preview/trial mode?  Or must it be activated?

No, it does not work in preview mode, it must be activated. Additionally, surveys will be in preview mode when you try to access a link in an email preview or test send.
