The Email & Digital Marketing system isn't really setup to ask custom questions like that outside of things like surveys and event registrations. If you're wanting to setup a sign-up form that can ask custom questions, or allow new sign-ups to submit longer form responses, while also automatically emailing specific distribution lists or members of the organization, then it'd need to be setup outside of the Constant Contact account.
