I am creating a new form in my account, and what shows when I choose the option of my new for is what I want, but when I copy the code and put it into my website, it includes an old form I had created and included in a different location. Any advice on how to tackle this issue?

Check to see if you have any other active forms in your account- active pop-up, fly-out, and banner forms will all show under the same universal code for an account's forms. If it's an older inline form, make sure it's disabled or at the very least that you've removed its unique inline form code from your site.
