How do I add an entry form to a survey for a respondents draw?

Hello @user616517 ,

There isn't really an "entry form" element for surveys. If you want to enter survey respondents into a drawing, you'd want to include required open answer questions for them to input their contact info and whatever pertinent details you need for the drawing.
