Nomination form

I need a nomination form that provides for multiple choice and short answers. I've tried using the survey or sign-up form, but neither work well.

Can you elaborate on what you're wanting to be able to do, or what about the survey pages doesn't fulfill the "nomination form" functionalities for open answer and multiple choices you're looking for?
